Integrated care hub plans outlined

The delivery of health care in Staffordshire, including in Cheadle, is set to change with the introduction of integrated care hubs. Each of these hubs will provide services for specific long-term conditions which will include rehabilitation, dementia services and mental health care. They will also cater for care home support, therapies and palliative care.
